Vagthol’s Residential Care Center employs a full-time Registered Nurse as manager of clinical services for 1-2 homes with support from the medical director, administrator, lead staff, and direct support professionals for all of its consumers.
As an exempt employee, the Nurse Manager is on-call 24 hours a day, 7 days a week.\
Qualifications
Have and maintain current Registered Nurse license
Have current CPR/First Aid and CPI certifications or obtain these certifications within 3 months of hiring
Be fingerprinted and obtain Live Scan clearance through Licensing
Have and maintain current biennial Medical Health clearance including passing TB screening
Have Direct Support Professional (DSP) 1 certificate within 1 year of date of hire
Have DSP 2 certificate within 2 years of date of hire
Have Adult Residential Facility certificate within 2 years of date of hire and maintain current certification
Knowledgeable and able to comply with applicable laws and regulations
Knowledgeable of the requirements for providing the type of care and supervision needed by the clients, including ability to communicate with such clients and their families and conservators
Complete personal record, including employment history, education, and references
Complete abuse reporting requirement statement
Have degree in Nursing and possess RN license in good standing
Have valid work authorization for employment in the US
Responsibilities
Duties include but are not limited to the following:
Develop and maintain a current Individual Healthcare Plan for each resident and Restrictive Health Conditions as needed
Routine monitoring of the medications prescribed for each resident
Conduct regular training to all Direct Support staff as it relates to the overall healthcare needs and wellness program, and certify qualified Vagthol’s staff on the administration of medication and monitoring of side-effects of medications prescribed to the residents
Develop and maintain good working relationships with the physician, nutritionist, and other members of the Inter-Disciplinary Team (IDTeam) of professionals for all consumers in collaboration with their special diet and treatment plan
Keep current all medical and dental appointments for the residents in coordination with the respective Administrator
Ensure that all Individual Program Plans address health issues in terms of residents’ daily needs and that these are updated at least annually or as needed
Ensure that all physicians’ orders shall be implemented as prescribed
Notify attending physician or medical specialist immediately of any sign of illness or marked change in condition
Control communicable and infectious diseases through:
Identification and assessment
Reporting to medical authorities
Implementation of appropriate protective and preventative measures
Participate as a member of the IDTeam, including the following areas:
Pre-admission evaluation
Program design and placement of the client at the time of admission to the facility
Periodic re-evaluation of the type, extent, and quality of services and programming as related to health issues
Discharge planning
Keep current all consents for psychotropic medications and ensure that residents are prescribed the least amount of psychotropic and neuroleptic medications as possible
Keep current all consents for medical/dental treatments and procedures
Provide Service Coordinators with quarterly and annual healthcare status reports for all residents
Obtain and/or maintain DDNA certification
Coordinate with the administrator to develop and maintain a safe environment that meets residents’ total healthcare needs
Assist in achieving and maintaining optimum health and prevention of illness for all residents
